Are looking to Recruit:
A HEALTH & WELL-BEING COMMUNITY REFERRAL LINK-WORKER (SOCIAL
PRESCRIBING MODEL)
PART TIME **APPLY ON THE WEBSITE** HOUR P.W (6 MONTH P/T
CONTRACT-PILOT SCHEME)
THE OVERALL AIM OF THE POSITION:
The Social Prescribing Service is a partnership initiative which aims
to link persons into activities within their communities to improve
their health and wellbeing with an overall aim of self-management.
APPLICANTS SHOULD HAVE:
* _A minimum of 3 years experience in a related field._
* _A relevant 3rd level qualification or equivalent_
* _Experience and knowledge of community based supports._
* _Proven experience of providing one-to-one & group support._
* _Access to own transport and full clean driver licence._
* _Interest in supporting people and in being a part of an exciting
pilot_
